Other ways we can help

  • Video visittelemedicine urgent care sees Florida patients the same day for colds, rashes, UTIs and refills.
  • Medical records — call the clinic where you were seen. Records are released to you on request and to anyone else only with your signed authorization; how we handle your information is set out in the privacy policy.
  • An accommodation for your visit — an interpreter, help with forms, extra time or assistance from the car — call the clinic before you arrive. See the accessibility statement.
  • Looking for a page you cannot find? The sitemap lists every page on the site, grouped the way the site is organised.
  • Work, school and program forms — completed at the visit; call the clinic for another copy. See doctor’s notes and medical forms.
  • Before your visitwhat to bring, insurance accepted, hours and directions.
  • Emailfrontdesk@urgentmed.us for general questions. Email is not secure for medical information, so please keep symptoms, results and insurance details for the phone or the visit.

Send us a message

A question, feedback, or a call back — not for medical questions or emergencies.

This form is not for medical questions or emergencies. Please do not include symptoms, test results, account numbers or dates of service — we confirm those by phone. If you need care today, walk in or call.

Billing & insurance

Billing questions are answered on the toll-free line, Monday to Friday, 9am–5pm. That is the billing team’s schedule, not the clinics’ — the clinics themselves are open seven days a week, but a billing question outside those hours is best left as a call-back request below. Have your statement handy: the account number on it lets the team find your visit without asking for anything else over the phone.

Good to know

  • Pay a balanceonline with the account number from your statement, or at either front desk by cash or card.
  • Insurance questions — the plan is billed first; your statement shows what remains after the plan pays and matches the explanation of benefits (EOB) the plan sends. Plans we bill: insurance accepted.
  • Work injuries and car accidents — authorized workers’ compensation visits are billed to the carrier and car accident (PIP) visits to the auto policy, never to you. If a statement reached you anyway, ask for a call back below and we will correct the claim.
  • No insuranceself-pay pricing is quoted before you are seen; HSA and FSA cards are accepted.

Employers & workers’ comp

One number for employer services — for employers, HR, safety managers, workers’ comp adjusters and nurse case managers: account setup, sending an injured employee, DOT and drug-testing programs, authorizations, visit reports and claim questions. Same-day answers from a local team — call, or use the form for anything that is not urgent.

New employers

Tell us what your company needs — DOT physicals, drug and alcohol testing, pre-employment physicals, screenings or injury care — and the occupational health team sets up the account, the billing and the reporting contact. How employer services work.

Injured employee today
