Florida camps, daycares and preschools ask for a completed physical — most often form DH 3040, the Florida Student Health Examination, together with an up-to-date immunization record. Both Broward clinics perform these as walk-ins, seven days a week, and the signed form leaves with you.
What the exam covers
- Height, weight, blood pressure and vital signs.
- Vision and hearing screening as the form requires.
- A head-to-toe examination by a licensed provider, including heart and lungs.
- A review of your child’s medical history, allergies, and any medication the camp or daycare will need to give.
- Review of the immunization record, and any vaccines that are due can be given during the same visit.
What to expect at your visit
- Walk in at either clinic — no appointment is needed.
- Bring the camp, daycare or school form, even if it is partly filled in.
- Bring the immunization record (Florida form DH 680 if you have it).
- The provider completes and signs the form during the visit.
- If a required vaccine is missing, it can usually be given the same day and recorded on the form.
Camp, daycare and school forms are not the same
A summer-camp form, a daycare or preschool form and a school-entry physical often ask for different information, and an athletic physical for a school team uses the separate FHSAA forms. Bring whichever document the organization gave you and the provider will complete that one — see school and sports physicals if your child needs an athletic clearance as well.

Both clinics are open seven days a week with on-site X-ray and laboratory — Davie on S University Drive and Plantation on Cleary Boulevard — serving Davie, Plantation, Cooper City, Weston, Sunrise, Southwest Ranches and the surrounding Broward County communities.
Insurance and self-pay
Camp and daycare physicals are often not covered by health insurance because a third party is requesting them. We accept most major plans where the exam is covered and quote self-pay pricing at the front desk before your child is seen.
