Doctor’s notes for work and school
- Sick notes — a note excusing absence for an illness or injury evaluated at the visit, with the dates and any restrictions.
- Work notes and restrictions — light duty, no lifting, or dates off work after an injury or illness; return-to-duty questions after a work injury are handled under fit-for-duty evaluations.
- School and college notes — absence excuses, and physical-education or sports restrictions after an injury.
- Jury duty and court — a physician’s statement when a medical condition evaluated here prevents service on the summoned dates.
- Travel and airline letters — fitness to fly, medication and medical-device letters for security screening.
Notes are written for what the provider evaluates and documents; UrgentMed does not backdate notes or excuse periods the provider did not assess.
FMLA certification
The Family and Medical Leave Act form (WH-380-E for your own condition, WH-380-F for a family member) asks a provider to certify a serious health condition and the expected duration and frequency of leave. UrgentMed completes it for conditions evaluated and treated here — an injury, an illness needing time off, a course of follow-up — and for intermittent leave tied to those visits. Long-term conditions managed by a specialist are best certified by that specialist, who has the full record; the provider will tell you which applies.
Disability parking permit (HSMV 83039)
Florida’s Application for Disabled Person Parking Permit requires a licensed physician, osteopath, podiatrist, chiropractor, optometrist, PA or APRN to certify a qualifying condition — inability to walk 200 feet without stopping, use of a wheelchair, crutches or another assistive device, severe lung or heart disease, or another listed impairment. Temporary permits (up to six months) are certified after an injury or surgery evaluated here; permanent permits need documentation of the underlying condition.
