A doctor’s note for work or school

FMLA paperwork (Family and Medical Leave Act certification)
The Family and Medical Leave Act form (WH-380-E for your own condition, WH-380-F for a family member) asks a provider to certify a serious health condition and the expected duration and frequency of leave. UrgentMed completes it for conditions evaluated and treated here — an injury, an illness needing time off, a course of follow-up — and for intermittent leave tied to those visits. Long-term conditions managed by a specialist are best certified by that specialist, who has the full record; the provider will tell you which applies.
Florida handicap parking permit form (HSMV 83039)
Florida’s Application for Disabled Person Parking Permit — form HSMV 83039, the form behind the handicap placard — requires a licensed physician, osteopath, podiatrist, chiropractor, optometrist, PA or APRN to certify a qualifying condition — inability to walk 200 feet without stopping, use of a wheelchair, crutches or another assistive device, severe lung or heart disease, or another listed impairment. Temporary permits (up to six months) are certified after an injury or surgery evaluated here; permanent permits need documentation of the underlying condition. Once the form is certified, you take it to a county tax collector’s office, which issues the permit itself.
