Employer Treatment Authorization — Send an Employee to UrgentMed

Use this employer treatment authorization page to authorize care for an employee at UrgentMed Davie or UrgentMed Plantation — a work injury, a DOT or pre-employment physical, a drug or alcohol test, or a fit-for-duty exam — and to tell us what the visit is for and where to send the report. The employee then walks in at either clinic, seven days a week.

Authorizing a visit today: call (800) 275-9630 and the clinic will take the authorization by phone or give you the fax number, or send it with the employee. Please do not email medical details about an employee; they are collected securely at the visit.

What this page will never ask you for. UrgentMed does not collect Social Security numbers, dates of birth, diagnoses or any other protected health information through this website. There is no form on this page for a reason: employee health information belongs in a system covered by a business associate agreement, not in a web page’s database. Authorization details reach us by phone or fax, and the clinical information is collected from the employee at the visit.

What to include

  • Company name, your name and role, and the phone number and email for the visit report.
  • The employee’s name and the reason for the visit — injury care, DOT physical, pre-employment physical, drug or alcohol test, fit-for-duty exam or screenings.
  • For a work injury: the date of injury and the workers’ compensation carrier or claim number if it has been reported. Not reported yet? We complete the DWC-25 at the first visit — see sending an injured employee.
  • Which clinic the employee will use, and whether the visit is billed to an existing employer account.

Where the employee is seen

Either clinic, walk-in, seven days a week: UrgentMed Davie at 5149 S University Drive off I-595, or UrgentMed Plantation at 10199 Cleary Boulevard near Nob Hill Road. Maps and directions.

What happens next

  • The employee is seen the same day, walk-in, at either clinic; X-ray, lab, drug and alcohol collections and EKG are on site.
  • The visit report, work-status note or DWC-25 goes to your designated contact the same day.

Frequently asked questions

Can the employee just walk in without an authorization?

Yes — for a work injury the employee can walk in and we will contact you for the authorization; for physicals and testing an authorization or employer account tells us what to perform and who to bill.

Where is the visit report sent?

To the contact you name — by fax or secure delivery the same day. Work-status notes and the DWC-25 go to the employer and carrier on record.

Do you need medical details from us?

No. Tell us the reason for the visit and the billing arrangement; medical history is collected from the employee at the clinic.
