Adjusters and nurse case managers need three things from a treating clinic: complete DWC-25 reporting on time, a work status that can be acted on, and a person who picks up the phone. That is how UrgentMed runs workers’ compensation care at its Davie and Plantation clinics.
Reporting
- A DWC-25 completed at every visit — initial and follow-up — with diagnosis, treatment plan, work status and next appointment.
- Care from the first visit through MMI at the same clinic, with medications dispensed and DME supplied in-clinic and billed to the claim — fewer outside vendors on the file.
- Work-status notes with specific, actionable restrictions and a re-evaluation date.
- Reports sent the same day to the adjuster and employer contact on file; delivery by fax or secure email as you prefer.
Coordination
- Authorization requests for treatment, imaging beyond on-site X-ray, and specialist referrals sent with the supporting documentation the first time.
- Referrals directed to the carrier’s preferred network when you tell us who is in it.
- Records requests fulfilled promptly with a signed authorization on file.
- Nurse case managers are welcome to coordinate follow-up scheduling with the clinic directly.
What we do on site
Provider evaluation, digital X-ray, laboratory testing and EKG, wound care and laceration repair, splinting and bracing, post-accident drug and alcohol testing, and return-to-work evaluations — at both clinics, seven days a week.
