Adoption and foster-care physicals
Florida foster-home licensing and most adoption home studies — domestic and international — require a physician’s statement that each adult in the household is in good health and able to care for a child, and often a TB test. International adoptions may need country-specific forms, notarized statements and additional tests. Bring the agency’s form; every adult household member can be seen at the same visit.
Study-abroad, university and exchange programs
Study-abroad offices and host universities require a health clearance, an immunization record and sometimes a TB test or titers. The form is completed and the printed immunization record issued at the visit; missing vaccines from the list stocked here are given the same day, and you are told which travel vaccines need a travel clinic.
Peace Corps, AmeriCorps, missionary and volunteer programs
The Peace Corps medical clearance uses its own physician forms and laboratory list; mission agencies, AmeriCorps and international volunteer organizations do the same. The provider completes the exam sections and orders the listed tests; results are attached when they return.
Job Corps, camp staff and other program forms
Job Corps, camp-counselor, coaching, childcare-staff and similar program physicals are completed the same way — bring the form and the list of required tests.
What to bring

Insurance and self-pay
Most licensing and program physicals are self-pay and the price is quoted at the front desk before you are seen; employer-ordered exams are billed to the employer account. Bring a photo ID and the form.
