Professional boxers, kickboxers and MMA fighters licensed in Florida need an annual pre-licensure physical by a physician, current HIV and hepatitis B and C results, and — at 40 and over — an EKG and brain MRI, under Florida Athletic Commission Rule 61K1-3.007. UrgentMed physicians complete the commission’s physical examination form (DBPR FSBC 20) and draw the required blood work and EKG on site at Davie or Plantation, walk-in.
What the Florida Athletic Commission requires
- Physical examination by a licensed M.D. or D.O. on Form DBPR FSBC 20, submitted with the license application.
- HIV — a negative test, valid for one year; hepatitis B and C — negative laboratory results, valid for one year.
- Dilated eye examination by an ophthalmologist or certified optometrist on Form BPR-0009-665 — arranged by referral; not performed at UrgentMed.
- Age 40 and over — an EKG within 12 months clearing the participant to compete, and a normal brain MRI (MRI by referral).
- Additional neurological or cardiac testing when the commission’s executive director requires it.
Amateur organizations and gyms use their own physical forms with similar content; bring the form and it is completed the same way.
What is done at the UrgentMed visit
- The FSBC 20 physical by a physician — history, neurological and musculoskeletal exam, vision, blood pressure and the fitness statement.
- Blood draw for HIV and hepatitis B and C, with results in a few business days and the report issued for your application.
- 12-lead EKG on site for participants 40 and over, or when the physician or commission requires one.
- Referral letters for the dilated eye exam and, when required, the brain MRI, so the packet is completed without delay.
Timing before a bout or license renewal
Results for HIV and hepatitis are valid for one year from the test date, so schedule the visit early enough for the laboratory report to return before the application deadline. Post-match physicals and suspensions are handled by the commission’s ringside physicians.
What to bring
- Form DBPR FSBC 20 printed, and your commission license application if you have started it.
- Photo ID, a list of medications, prior fight-medical records if you have them, and results from another U.S. jurisdiction if you are transferring them.

Insurance and self-pay
Most licensing and program physicals are self-pay and the price is quoted at the front desk before you are seen; employer-ordered exams are billed to the employer account. Bring a photo ID and the form.
