- Pick the clinic — Davie on S University Drive or Plantation on Cleary Boulevard — and choose a time.
- Enter your name and contact details; your medical history is taken at the clinic, not on the web.
- You get a text when it is close to your time — then head in. Bring your photo ID and insurance card, or your employer’s authorization for a workers’ comp or occupational visit.
Walk in any time
Online check-in is a convenience, not a requirement. If you are already nearby, walk in — the front desk registers you and you are seen in order. Both clinics are open evenings and weekends; today’s hours are on each location page.
What to bring
- Photo ID and your insurance card — see the plans we accept, or self-pay pricing if you are uninsured.
- A list of current medications and allergies.
- For a work injury: your employer’s name, the date of injury and the claim or authorization if you have one.
- For a car accident: the crash date and your auto policy — Florida PIP requires care within 14 days.
- For a DOT physical: your driver’s license, medication list and any glasses, hearing aids or CPAP compliance report.

Prefer to be seen from home?
Telemedicine urgent care sees Florida patients the same day for colds, rashes, UTIs, prescription refills and follow-ups. If an exam, X-ray or test is needed, the provider sends you to the clinic with your place already held.
Sending an employee?
Employers do not need to check an employee in. Send the treatment authorization and the employee can walk in at either clinic; results and work-status forms are reported the same day. Employer services has the full list. Guides for patients and employers are on the resources page.
