Chubb Workers’ Comp Claims: Same-Day Injury Care in Davie & Plantation

Chubb workers’ comp claims are treated at UrgentMed at both clinics — Davie and Plantation — seven days a week, no appointment needed. Authorized care is billed to Chubb, not to you — with the DWC-25 report completed at every visit and sent to the adjuster the same day.

How Chubb workers’ comp claims work at UrgentMed

Chubb is the world’s largest publicly traded property and casualty insurer; its workers’ compensation claims are administered directly and through ESIS, and it also writes personal auto coverage — so the same clinics handle Chubb PIP claims after a crash.

Chubb writes the workers’ compensation policy itself, so the adjuster, the authorization and the DWC-25 all sit with Chubb. If your employer uses a third-party administrator for Chubb claims, tell the front desk the administrator’s name as well.

Walk in any day and tell the front desk it is a work injury. We contact your employer or the Chubb adjuster for authorization while you are being seen, treat the injury at that visit with X-ray and lab on site, and send the DWC-25 and your work-status note to Chubb and your employer the same day. Authorized care is billed to Chubb — not to you.

Cards that do not say Chubb

Some plans on Chubb coverage carry a different name on the front of the card — ESIS. Those are billed to Chubb all the same, so bring the card you have and the front desk reads the network from it.

The full claim path — what a visit includes, the injuries treated, how authorization works in Florida and care through maximum medical improvement — is on the workers’ compensation page; the adjuster and case-manager page covers reporting and work status, and every carrier we bill is listed on the workers’ compensation carriers page.

Chubb auto policies after a crash

Chubb also insures drivers in Florida, so the same clinics treat Chubb policyholders after a crash and bill personal injury protection directly — see the car accident care page for what the visit involves.

Come in within 14 days of the crash — Florida’s no-fault law requires initial care inside that window for PIP medical benefits to apply. Bring the policy or claim number, the date of the accident and a photo ID. The visit includes a provider exam, digital X-ray on site and, where it applies, the emergency-medical-condition determination that decides whether the $10,000 or the $2,500 PIP medical limit is available. We bill Chubb PIP directly; if PIP runs out or is denied while an attorney is handling your claim, treatment can continue under a letter of protection.

Frequently asked questions

Does UrgentMed treat Chubb workers’ compensation claims?

Yes. Employees whose claims are handled by Chubb are seen at both clinics, and authorized care is billed to Chubb — you do not pay for authorized treatment.

Is ESIS the same as Chubb?

For a visit here, yes — ESIS runs on Chubb coverage, so it is handled the same way at check-in and billed to Chubb. Bring the card you have; the network named on the back is what decides it, not the logo on the front.

Does Chubb pay the clinic directly?

Yes. Chubb writes the policy, so authorized treatment is billed to Chubb and not to you. If your employer uses an administrator for Chubb claims, tell the front desk that name too.

Do I need authorization before I come in?

Come in as soon as you can and tell the front desk it is a work injury; we contact your employer or the adjuster for authorization while you are being seen. Employers with a standing account are pre-authorized.

What does UrgentMed send to Chubb?

A completed DWC-25 after every visit — diagnosis, treatment plan and work status — plus records, referral requests and authorization requests, sent the same day to the adjuster and employer contact on file.

Does UrgentMed accept Chubb PIP claims?

Yes. We treat drivers and passengers with Chubb policies at both clinics and bill Chubb personal injury protection directly.
