How to send an injured employee
- Call the clinic and tell us the employee is coming, or send them with a note naming your company and contact.
- Or complete the treatment authorization so the visit is authorized before the employee arrives.

What you receive
- A completed DWC-25 from every visit — diagnosis, treatment plan, work status — sent to you and your carrier.
