Nursing school physicals, and the matching packets for dental, EMS, medical-assistant and allied-health programs — and for the hospitals and practices that hire their graduates — all come down to the same bundle: a physical, proof of immunity, a TB test, hepatitis B and often a drug screen. Nursing school and clinical-program physicals
Every program has its own form, and most have a deadline before clinicals start. Bring it and the provider works through it line by line. (For a K-12 student’s form — the DH 3040 or a sports form — see school and sports physicals.)
- History and physical examination, with height, weight, vision and vital signs recorded where the form asks.
- Titers for measles, mumps, rubella, varicella and hepatitis B, drawn on site; when a titer shows no immunity, the vaccine is given once the result is back.
- TB testing — two-step PPD or the single-visit blood test (IGRA) most programs now accept.
- Hepatitis B series or booster, Tdap or Td, and the seasonal flu shot the program requires.
- Drug screen — 10-panel with chain of custody when the program or hospital requires it.
- The form completed and signed, plus a printed immunization record for your file.
Healthcare-worker and hospital onboarding physicals
New hires at hospitals, surgery centers, dental offices, home-health agencies and labs need the same items on an employer timeline. Employers can set up an account so employees are sent with a simple authorization and results are reported to HR or employee health; the visit can include a pre-employment physical, a respirator fit test for N95 use, and other screenings the role requires.
Nursing and healthcare physicals — start before the deadline
- Titers take a few business days; if a vaccine is needed, some series need weeks between doses. Start early.
- A two-step TB skin test needs four visits over one to three weeks; the IGRA blood test needs one — ask which your program accepts.
- Vaccines your records show are due, and the flu shot, can be given at the first visit and recorded immediately.
- The signed form is issued when every required item is complete; interim documentation is provided when a program needs proof that testing is under way.

- The program’s or employer’s form, and its list of required vaccines and tests.
- Any prior immunization records or titer results — in any language — to avoid repeating doses.
- Photo ID; the employer authorization if your employer is paying.

Student physicals are usually self-pay and itemized — exam, each titer, TB test, each vaccine — so you pay only for what your program still needs. Employer-ordered onboarding is billed to the employer account.
