Florida requires every law enforcement, corrections and correctional probation officer to pass a physical examination before certification, documented on the FDLE Physician’s Assessment (Form CJSTC-75). UrgentMed completes it — and the academy, agency and security-licensing forms that go with it — as a walk-in visit at Davie or Plantation, with the EKG, lab work, X-ray and drug testing those forms often require on site.
FDLE CJSTC-75 physician’s assessment
The Criminal Justice Standards and Training Commission requires the assessment to be completed by a licensed physician, physician assistant or advanced practice registered nurse. You bring the CJSTC-75 (and the CJSTC-75A patient information sheet you complete beforehand); the provider takes your history, examines you, records vision and hearing, and signs the assessment for the academy or the hiring agency.
- Police academy entrance — Broward College Institute of Public Safety and other Florida training centers.
- Agency pre-hire and reactivation physicals for police, sheriff’s office, corrections and probation officers.
- Drug testing required by the agency’s background investigation, collected with chain of custody at the same visit.
- Agency-specific additions when requested — EKG, blood work, chest X-ray, spirometry, TB test and immunizations.
Corrections, probation and detention officers
The same CJSTC-75 assessment applies to corrections and correctional probation officers, and county detention agencies typically add a TB test and immunization review. Bring the agency’s packet and it is completed in the same visit.
Security officer licensing (Class D and Class G)
Florida’s armed security officer (Class G) application, FDACS-16008, includes a medical section for a licensed physician to complete, and many security employers and training schools require a fitness-for-duty medical statement for unarmed (Class D) officers as well. Bring the application or the employer’s form; the medical section is completed by a physician at the visit.
What to bring
- The CJSTC-75 and completed CJSTC-75A, or the agency, academy or FDACS form you were given.
- Photo ID, glasses or contacts, hearing aids if you use them, and a list of medications.
- Any specialist letters for a condition the form asks about.

Insurance and self-pay
Most licensing and program physicals are self-pay and the price is quoted at the front desk before you are seen; employer-ordered exams are billed to the employer account. Bring a photo ID and the form.
