OSHA’s crane standard (29 CFR 1926.1427) and the ASME B30 series set physical requirements for crane operators, and construction, warehouse and port employers apply similar fitness standards to forklift, aerial-lift and heavy-equipment operators. Crane operator physical (ASME B30 / OSHA 1926.1427)
The ASME B30 physical qualifications that certifying bodies such as NCCCO and employers require include:
- Vision of at least 20/30 in one eye and 20/50 in the other, with or without correction, plus normal depth perception, field of vision and the ability to distinguish colors when signaling depends on it.
- Hearing adequate for the operation, with or without a hearing aid.
- Sufficient strength, endurance, agility, coordination and reaction speed for the equipment.
- No evidence of physical defects or emotional instability that would create a hazard, and no seizure disorder or loss-of-consciousness history that is not medically controlled.
- A negative drug test where the employer or certifying body requires one.
The provider records the findings on the certifying body’s or employer’s form and issues a fitness statement; when the employer requires a full pre-employment physical the two are combined.
Forklift, aerial lift and heavy-equipment operators
OSHA does not prescribe a medical exam for powered industrial truck operators, but employers must ensure operators are capable of safe operation, and many require a fitness exam at hire and after an incident. The exam covers vision, hearing, mobility, medication review and any condition that affects alertness, with a fitness-for-duty determination sent to the employer.

Set up an employer account so operators are sent with an authorization and results go to your safety manager the same day; pair the exam with respirator fit testing, OSHA medical surveillance or DOT physicals for drivers in the same visit.

Operator physicals are billed to the employer account or self-pay; the price is quoted before the visit. Bring a photo ID and the employer or certifying body’s form.
