A fit-for-duty or return-to-work evaluation answers one question for the employer and the employee: can this person safely do this job, with or without restrictions, right now? Both Broward clinics perform them as walk-ins, with the work-status form completed during the visit.

When an evaluation is needed

  • Returning after an injury or illness, whether or not it was work-related.
  • Returning after a medical leave, when the employer requires clearance before resuming duties.
  • A change in job duties, or a concern about an employee’s ability to perform essential functions safely.
  • Documenting light-duty or modified-duty restrictions, and later clearing them.

What the evaluation covers

  • A review of the injury or condition, treatment to date and any specialist recommendations.
  • A focused physical examination matched to the job’s essential functions — lifting, standing, climbing, driving, the use of equipment.
  • A clear written determination: fit for full duty, fit with specific restrictions, or not yet fit, with a re-evaluation date.
  • The employer’s own form, or a work-status note, completed and signed during the visit.

Related workplace exams

An evaluation after a work injury is usually part of workers’ compensation care, where the same visit updates the DWC-25. A first-day clearance for a new hire is a pre-employment physical; commercial drivers need a DOT physical; and employers who require it can pair the evaluation with drug and alcohol testing.

What to bring

  • A photo ID and the employer’s form or written request, including the job description if there is one.
  • Records from any treating physician or specialist, and a list of current medications.
  • Any restrictions previously issued, so they can be updated rather than repeated.

We accept most major insurance plans, workers’ compensation and auto (PIP) claims, and offer self-pay pricing quoted at the front desk before you are seen. Bring a photo ID and your insurance card.

Frequently asked questions

Do I need an appointment?

No. Both clinics perform fit-for-duty and return-to-work evaluations as walk-ins seven days a week.

Will the form be completed the same day?

Yes. The provider completes and signs the employer’s form or a work-status note during the visit, and it can be sent directly to the employer on request.

What if I cannot yet return to full duty?

The provider documents specific restrictions — what you can and cannot do — and a re-evaluation date, so the employer can decide about modified duty.

Is this the same as a workers’ compensation visit?

It can be part of one. After a work injury, the return-to-work determination is made at a workers’ compensation visit and recorded on the DWC-25. Evaluations after non-work injuries or leaves are separate.

Can the employer request the evaluation?

Yes. Employers send the employee with a written request or their own form; results are reported to the employer contact named on the request.
