Physical exams: choose the one you need

Not sure which one applies to you?

• A school, college, sport or team is asking — you need a school or sports physical (student physicals included), on the FHSAA EL2 form for athletics (the EL3 is the parents’ consent).
• A camp, daycare or preschool is asking — you need a camp or daycare physical, usually on Florida form DH 3040 together with an immunization record.
• An employer is asking before you start — you need a pre-employment or work physical, often paired with a drug screen in the same visit.
• You drive commercially — you need a DOT/CDL physical performed by a certified medical examiner.
• USCIS is asking — you need an immigration medical exam (Form I-693) performed by a designated civil surgeon.
• A surgeon or anesthesiologist is asking — you need a pre-operative medical clearance, which may include an EKG and blood work.
• A nursing, dental or clinical program — or a hospital HR department — is asking — you need a clinical-program or healthcare-worker physical with titers, TB test and hepatitis B.
• A police academy, fire academy, agency or licensing board is asking — see law enforcement and security, firefighter, school bus driver, merchant mariner (CG-719K), crane and forklift operator, scuba and boxing and MMA physicals.
• An adoption agency, study-abroad office or service program is asking — adoption, foster-care and program physicals.
• Nobody is asking — you just want a yearly check — an annual or executive physical.
• You need paperwork, not an exam — doctor’s notes, FMLA and disability forms.

Whatever the form, the tests it asks for are on site: blood work, TB testing, blood pressure evaluation, EKG, spirometry, X-ray, vaccines and drug testing — so the visit that starts the form usually finishes it.
