Patient Forms & What to Bring — Before Your Visit to UrgentMed

Registration is completed at the front desk when you arrive, at either UrgentMed Davie or UrgentMed Plantation, so there is nothing to print. This page tells you what to bring for each kind of visit so the paperwork is finished during the visit — a work note, a DWC-25, a signed school form or a DOT medical card leaves with you.

Every visit

• A government photo ID.
• Your insurance card — see the plans we accept. No insurance? Self-pay pricing is quoted before you are seen.
• A list of current medications, allergies and your primary care physician, so the visit summary reaches the right office.
• For patients under 18, a parent or legal guardian to sign consent. If a guardian cannot come, call the clinic first.

Work injuries (workers’ compensation)

• Your employer’s name and phone number, the date and a short description of the injury.
• The claim number or carrier name if your employer has already reported it — if not, we start the workers’ compensation paperwork here. Read what to do after a work injury.
• Employers: the treatment authorization tells us what the visit is for and where to send the report.

Visits billed to auto insurance

• The crash date and your auto insurance policy (yours, or the vehicle owner’s). Florida PIP covers the first visit only if it happens within 14 days of the crash — details on the car accident care page.
• The police report or exchange of information if you have it; it is not required to be seen.

Physicals and clearances

• DOT physical: driver’s license, medication list, glasses or contacts, hearing aids, and a CPAP compliance report if you use one.
• School and sports physicals: the FHSAA EL2 form with the parent sections signed, or the school’s form; immunization record for new enrollments.
• Camp and daycare physicals: the Florida DH 3040 is completed here; bring the immunization record.
• Immigration physical (I-693): passport or government ID and every vaccination record you have.
• Pre-employment physicals and drug tests: your employer’s paperwork or chain-of-custody form if one was issued.
• Nursing and healthcare program physicals, police (CJSTC-75), firefighter, merchant mariner and dive physicals: the program’s form, plus prior titers or immunization records.
• Surgical clearance: the surgeon’s request and the date of surgery.

Forms we complete for you

Work and school excuse notes, return-to-work and light-duty notes, FMLA certification, disability parking permit forms, jury duty letters and program physical forms are completed during the visit — see doctor’s notes and medical forms. Workers’ compensation visits leave with the DWC-25; employer visits are reported to the designated contact the same day.

Records and privacy

Please do not send medical information by email or through this website. Records are released to you on request, and to anyone else only with your signed authorization — call the clinic where you were seen. How we handle your information is described in the privacy policy.
