Zurich North America Workers’ Comp Claims: Same-Day Injury Care in Davie & Plantation
Zurich North America workers’ compensation claims are treated at UrgentMed at both clinics — Davie and Plantation — seven days a week, no appointment needed. Authorized care is billed to Zurich North America, not to you — with the DWC-25 report completed at every visit and sent to the adjuster the same day.
How care works when Zurich North America handles the claim
Zurich North America writes workers’ compensation for mid-size and large employers, including construction and hospitality programs common in South Florida.
- Report the injury to your employer and come in — walk-ins are seen the same day, seven days a week. Bring your employer’s name and, if you have it, the claim number.
- Authorization. We contact your employer or the Zurich North America adjuster to confirm authorization while you are being seen. Employers with a standing account are pre-authorized.
- Evaluation and treatment at the visit — provider exam, X-ray, lab and EKG on site, wound care and splinting
- Reporting. The DWC-25 (Florida’s treatment and work-status form) is completed at every visit and sent to Zurich North America and your employer the same day, with a work-status note for you.
- Follow-up through MMI. Medications and braces are dispensed in the clinic under the claim, follow-ups are scheduled here, and a return-to-work evaluation clears you when you are ready.

Injuries we treat under Zurich North America claims
- Sprains, strains and fractures of the back, shoulder, knee, wrist, ankle, hand and foot — X-rayed and splinted at the visit.
- Cuts, lacerations and puncture wounds, crush injuries to fingers and hands, wound care and dressing changes.
- Slips, trips and falls; lifting, overexertion and repetitive-motion injuries; minor burns and chemical exposures to the skin; eye injuries and foreign bodies.
- Post-accident drug and alcohol testing at the same visit when the employer’s policy requires it, with chain-of-custody collection.
What happens after the first visit
Follow-up visits are scheduled here, not at another facility. At each one the provider updates the work-status note and DWC-25, adjusts restrictions as you improve, and dispenses medication or supplies under the claim. If imaging beyond X-ray or a specialist is needed, the referral request goes to the Zurich North America adjuster with the records attached; referrals stay within the carrier’s preferred network when we are told who is in it. When you reach maximum medical improvement the final DWC-25 records it and, if needed, an impairment referral is arranged.
