How to send a client

  1. Tell the client to walk in at either clinic, or reserve a place online, with a photo ID, the current Form I-693 unsigned, and every vaccination record they have — records in another language need a reliable English translation.
  2. If your office wants a copy of the completed form, give the client your office’s contact details for the release.
  3. The civil surgeon examines the client, the labs are drawn, and the stocked vaccines that are due can be given; the client signs the I-693 in front of the civil surgeon.
  4. When the results are back, the civil surgeon completes and signs the form and seals it in the USCIS envelope. The client files it unopened with the I-485.
